How should a community go about addressing the most challenging health issues facing its residents? Health departments across the country are working to address these issues. An emerging best practice involves focusing on social determinants of health and aligning with the community health improvement plan. This is the approach that the Ogden Civic Action Network and the Weber-Morgan Health Department has utilized for the past five years. The Weber-Morgan Health Department leads three committees that align with the Community Health Improvement Plan. The three areas of focus are mental health, substance use, and healthy living. Interventions are funded by OgdenCAN to address these needs. So far nine interventions have been funded totaling $249,313.
